PAYMENT POLICY UPDATE 2026

• Payment is due at the time of service for all therapy appointments.
• A credit card is required to be kept on file, unless you have a Medicaid Managed Health Plan.
• Deductibles, co-pays and/or co-insurance amounts owed will be charged at the time of your visit, or within a few days after your appointment.
• You are responsible if your insurance does not pay.
• Missed appointments or cancellations within 24 hours will incur a $99 cancellation fee.
• All account balances must be current and paid before your next visit.
• Please notify us promptly of any insurance changes.
